If hot flashes are wrecking sleep, gabapentin can sound like a two-for-one answer. Fewer night sweats, more sleep. That is the hope.
The evidence is more specific. In a 197-participant randomized trial of menopausal hot flashes, gabapentin 900 mg/day reduced hot flash scores by 51% over 4 weeks, compared with 26% on placebo. [1] In another 59-woman randomized trial, gabapentin 900 mg/day reduced hot flash frequency by 45% and a composite hot-flash score by 54% at 12 weeks, compared with 29% and 31% for placebo. [2]
That makes gabapentin a real nonhormonal option. It does not make it a casual sleep supplement.
What gabapentin can actually answer
Gabapentin is not estrogen, progesterone, or a menopause hormone therapy. It is a prescription medicine with neurologic effects, and use for menopausal vasomotor symptoms is off-label. The clinical question is not whether it "knocks someone out." The clinical question is whether it reduces hot flashes or night sweats enough to justify the side-effect and safety tradeoffs.
The 2023 Menopause Society nonhormone statement lists gabapentin as a Level I recommended nonhormonal option for vasomotor symptoms. The same statement says hormone therapy remains the most effective treatment for vasomotor symptoms and that nonhormonal options are important when hormone therapy is contraindicated, not preferred, or not appropriate for the patient. A JAMA menopause review put the comparison in practical terms: systemic estrogen reduces vasomotor symptom frequency by about 75%, while nonhormonal therapies such as some selective serotonin reuptake inhibitors or serotonin-norepinephrine reuptake inhibitors, gabapentin, and others reduce frequency by about 40% to 65%. [4] In plain language, gabapentin can be meaningful, but it is not simply the same decision as estrogen with a different name.
For a structured menopause assessment, the useful first split is symptom target: hot flashes and night sweats, insomnia without heat symptoms, mood symptoms, pain, or medication-related waking. Gabapentin belongs mostly in the hot-flash and night-sweat category.
How much benefit was measured
The two core menopausal trials tested gabapentin 900 mg/day and found benefit beyond placebo. In the 4-week trial, 193 of 197 participants completed the study, and the week-4 hot-flash score fell 51% with gabapentin versus 26% with placebo. Early dizziness, unsteadiness, and drowsiness were more common in the gabapentin group, but these adverse effects improved by week 2 and returned to baseline by week 4 in that study. [1]
In the 12-week postmenopausal trial, gabapentin 900 mg/day reduced hot-flash frequency by 45% and composite score by 54%, compared with 29% and 31% on placebo. Four women assigned to gabapentin withdrew because of adverse events, compared with one on placebo. At least one adverse event was reported by 50.0% of the gabapentin group and 27.6% of the placebo group. [2]
A JAMA systematic review of nonhormonal therapies estimated that gabapentin reduced daily hot flashes by about 2.05 more episodes per day than placebo across two trials, with a 95% confidence interval from 1.30 to 2.80 fewer daily hot flashes. The review also warned that these effects were smaller than estrogen's effect and were most relevant for highly symptomatic women who could not take estrogen or preferred not to. [5]
A model-based meta-analysis found a rapid onset signal for gabapentin and other nonhormonal treatments, but it should be read cautiously because it pooled different trials rather than replacing direct head-to-head decisions. [6]

Why it is not just a sleep medicine
Night sweats and insomnia often travel together, so nighttime dosing can sound intuitive. But sedation is not the same as vasomotor symptom control.
DailyMed labeling for gabapentin warns about somnolence, dizziness, and impaired ability to drive or operate complex machinery until a person knows how the medication affects them. It also says dose selection in older adults should be cautious because kidney function is more likely to be reduced, and gabapentin dosing should be adjusted in reduced renal function. [7]
The label also warns about respiratory depression, especially with opioids, other central nervous system depressants, or underlying respiratory impairment. It says gabapentin should be tapered gradually over at least 1 week when reducing, discontinuing, or substituting therapy. [7]
That matters after menopause because the same night can include hot flashes, sleep disruption, alcohol, antihistamines, sleep medicines, bathroom trips, balance issues, and untreated sleep apnea. A medication that helps one part of the night can still make another part riskier.

Red flags and safety checks
Red flags include trouble breathing, swelling of the face or throat, severe rash, suicidal thoughts, severe confusion, repeated falls, heavy sedation, new inability to drive safely, or symptoms after combining gabapentin with opioids, alcohol, benzodiazepines, sleep medicines, or other sedatives. DailyMed labeling describes serious hypersensitivity reactions, mood and suicidality warnings, central nervous system depression, respiratory depression, renal dosing, and tapering cautions. [7]
The prescription check should include kidney function, age, fall history, driving or caregiving duties, alcohol, opioids, benzodiazepines, sleep medicines, antihistamines, sleep apnea or lung disease, mood history, and the current symptom log. It should also ask whether the symptoms are truly hot flashes or night sweats rather than fever, infection, thyroid disease, medication effects, panic symptoms, or another cause.

- Does gabapentin fit better than hormone therapy, fezolinetant, a selective serotonin reuptake inhibitor or serotonin-norepinephrine reuptake inhibitor, oxybutynin, clonidine, or no medication?
- Do kidney function, dizziness, falls, alcohol, opioids, antihistamines, benzodiazepines, sleep medicines, sleep apnea, or lung disease make it risky?
- Is the symptom target hot flashes, night sweats, insomnia, anxiety, pain, or medication-related waking?
- Should dosing be focused at night, split through the day, adjusted for kidney function, or avoided?
- What benefit should I expect by 4 to 8 weeks, and when should we stop or taper?
Bottom line
Gabapentin has randomized-trial evidence for menopausal hot flashes, including a 51% hot-flash score reduction over 4 weeks in one trial and a 45% frequency reduction over 12 weeks in another. [1] [2] It belongs in the prescription, off-label, clinician-screened nonhormonal category, especially when sedation, dizziness, kidney function, opioids, respiratory risk, mood symptoms, and fall risk matter.
