If hot flashes and night sweats are intense enough to disrupt sleep, clothing, work, or daily confidence, it is reasonable to ask a second question: do they also say anything about bone risk?
A 2024 systematic review and meta-analysis found that vasomotor symptoms were associated with low bone mineral density in postmenopausal women. The pooled low-bone-density odds ratio was 1.54, and women with moderate or severe symptoms had an odds ratio of 1.79 compared with women without symptoms. [1]
That is a useful risk clue. The evidence is limited because the included studies were observational, so the result is not a diagnosis and does not mean hot flashes caused the bone loss. It means bone density belongs in the same clinician-led review as symptom severity, menopause timing, fracture history, medicines, falls, and whether hormone or nonhormone treatment is the right fit.
What the 2024 meta-analysis found
The 2024 review included 20 observational studies in the qualitative analysis and 12 in the quantitative analysis, with 49,659 women. The authors looked at fractures, low bone mineral density, and mean differences in lumbar-spine and femoral-neck bone mineral density between women with and without vasomotor symptoms. [1]

The most important boundary is the fracture result. The study found a low-bone-density association, not a clear pooled fracture-risk increase. A woman with severe hot flashes should not be told that symptoms predict a fracture. She should be told that symptoms may be one more reason to review bone risk carefully.
Why hot flashes and bone loss can travel together
Vasomotor symptoms are the menopause category that includes hot flashes and night sweats. Bone loss after menopause is also tied to hormonal change, especially the estrogen drop around the transition.
The 2024 review describes several possible shared pathways, including estrogen change, sympathetic nervous system activity, cortisol after hot flushes, endothelial dysfunction, inflammatory markers, and oxidative stress. [1]
Those mechanisms are plausible, but they do not turn symptoms into a lab test. They explain why the association makes biological sense and why severe symptoms should not be dismissed as a comfort issue only.
The practical translation is this: if vasomotor symptoms are frequent or severe, the visit should not stop at "What can reduce hot flashes?" It should also ask, "Do my age and risk factors mean I need bone-density screening earlier than I expected?"
When symptoms should trigger a bone-risk conversation
U.S. Preventive Services Task Force recommends osteoporosis screening for women 65 or older. It also recommends screening postmenopausal women younger than 65 when they have 1 or more risk factors and clinical risk assessment shows increased fracture risk. [2]
Hot flashes are not named as a stand-alone U.S. Preventive Services Task Force screening rule. But moderate or severe symptoms can belong in the broader risk conversation, especially when they appear with other risk factors. This is a better fit for a clinician-led screening discussion when the symptom pattern is not isolated and the rest of the history also raises fracture risk.

This is where a symptom article and a bone-density article meet. If hot flashes are mild and occasional, the bone plan may be routine. If symptoms are severe, menopause happened early, or other fracture-risk factors are present, bone-density timing deserves a more direct question.
What a bone-density scan can and cannot answer
A bone-density scan can show whether bone mineral density is normal, osteopenia, or in the osteoporosis range. It does not capture every fracture-risk driver by itself.
The Bone Health and Osteoporosis Foundation clinician guide defines osteoporosis by bone mineral density at the hip or lumbar spine at or below a T-score of -2.5. It also emphasizes that many fractures occur in people whose T-scores are better than -2.5 because age, falls, prior fracture, frailty, and bone quality also matter. [3]
That matters for women with hot flashes because symptoms can make the conversation feel like a menopause-treatment decision only. Bone risk has its own decision tree:

The point is not to medicalize every hot flash. The point is to avoid missing bone risk when symptoms are severe or when the rest of the history raises concern.
Treatment choices should stay separated
Hot-flash treatment and bone-risk treatment overlap, but they are not the same decision.
The 2022 Menopause Society hormone-therapy statement says hormone therapy remains the most effective treatment for vasomotor symptoms and has been shown to prevent bone loss and fracture. It also says personalization, shared decision-making, route, dose, duration, age, time since menopause, and whether a progestogen is used all matter. [4]
That makes hormone therapy a possible bridge for some women: bothersome hot flashes near menopause plus a bone-prevention reason can belong in the same discussion when there are no contraindications. [4]
But the bridge has limits. Established osteoporosis, prior hip or spine fracture, very high fracture risk, or a high FRAX estimate usually needs an osteoporosis-specific treatment discussion, not only a menopause-symptom plan. [3]
Nonhormonal hot-flash treatments also need clean boundaries. Fezolinetant, elinzanetant, antidepressant-class options, gabapentin, oxybutynin, behavioral approaches, and sleep-specific care can be appropriate for vasomotor symptoms in selected patients. They should not be assumed to protect bone unless the specific evidence supports that bone outcome.
Red flags that should not wait
Seek prompt clinician review if hot flashes or night sweats come with symptoms that do not fit a typical menopause pattern, or if bone-risk red flags are present.

Red flags do not mean medication is the default answer. They mean the first job is diagnosis and risk stratification.
What to ask your clinician
Ask questions that separate symptom relief from bone protection:
- Do my hot flashes or night sweats sound like vasomotor symptoms, or should we evaluate another cause of sweating or flushing?
- Given symptom severity and my risk factors, should I have bone-density screening before 65?
- Which risk factors matter most for me: early menopause, prior fracture, body weight, smoking, alcohol, parental hip fracture, rheumatoid arthritis, medicines, cancer treatment, falls, or something else?
- If I get a bone-density scan, will we calculate fracture risk with FRAX?
- If hormone therapy is discussed for hot flashes, is bone prevention part of the reason, and what contraindications change that plan?
- If I choose a nonhormonal hot-flash treatment, what separate plan protects bone if my fracture risk is elevated?
- What would change follow-up: a new fracture, a T-score near -2.5, high FRAX risk, starting or stopping hormone therapy, or medication-related bone loss?
Bottom line
Hot flashes and night sweats can be more than a comfort problem after menopause. A 2024 meta-analysis linked vasomotor symptoms, especially moderate or severe symptoms, with lower bone mineral density.
The evidence does not prove hot flashes cause bone loss, and it did not show a clear pooled fracture-risk increase. The useful takeaway is narrower: severe vasomotor symptoms should prompt a better bone-risk conversation, especially when other screening or fracture-risk factors are present.
