Fatigue around menopause is real. A vitamin aisle can still be the wrong first stop.
MedlinePlus lists trouble sleeping as a menopause-transition symptom and says that, over time, lack of sleep can lead to fatigue and memory problems. [1]
That means the first question is not "Which vitamin gives me energy?" It is: what is draining energy? Night sweats, insomnia, heavy or irregular bleeding, under-eating, thyroid disease, anemia, vitamin B12 deficiency, low vitamin D, magnesium deficiency, depression, medication effects, alcohol, pain, and sleep apnea can all feel like "menopause fatigue."
Supplements are useful when they match the problem. They are wasteful, and sometimes risky, when they replace the workup.
Start with the cause of fatigue

The useful vitamins are deficiency-dependent
Vitamin and mineral supplements are not all the same. Some are replacement therapy for a deficiency. Some are bone-health support. Some are mostly marketing when the level is already normal.

The evidence limit is the main message: there is not enough evidence to treat "menopause fatigue" as one vitamin-responsive condition.
Vitamin B12: useful when status is low, oversold when it is not
National Institute for Health and Care Excellence guidance lists unexplained fatigue as a common possible symptom of vitamin B12 deficiency, but also says symptoms and signs can vary and are often not exclusive to vitamin B12 deficiency. It also says not to rule out vitamin B12 deficiency solely because anemia or macrocytosis is absent. [2]
Risk context matters. The same guidance lists low animal-source-food intake, lack of fortified foods, autoimmune gastritis, celiac disease or another autoimmune condition, metformin, histamine-2 receptor blockers, proton pump inhibitors, gastrointestinal surgery, and nitrous oxide use as risk factors. [2]
That does not mean everyone with menopause fatigue needs high-dose vitamin B12. If vitamin B12 deficiency is unlikely, the guidance says to investigate other causes of symptoms and consider repeat testing if symptoms are still present 3 to 6 months later. [2]
The practical question is: is vitamin B12 low or plausibly low? If yes, replacement can be important. If no, it is unlikely to be the main answer.
Iron: check before taking it
Iron deficiency anemia is a classic fatigue cause, and it can overlap with the menopause transition when bleeding is heavy, prolonged, or irregular.
Office on Women's Health says iron-deficiency anemia often develops slowly. As it worsens, symptoms can include fatigue, weakness, dizziness, headaches, low body temperature, pale or yellow skin, rapid or irregular heartbeat, shortness of breath, chest pain with physical activity, brittle nails, and pica. [3]
It also says diagnosis may include a complete blood count and iron-level testing, and that if the complete blood count shows anemia, another blood test will likely measure iron levels to confirm iron-deficiency anemia. [3]
The safety boundary is direct: do not take iron pills without first talking to a clinician. Office on Women's Health says extra iron can damage the liver, heart, and pancreas, and advises trying not to get more than 45 mg a day unless a clinician prescribes more. [3]
For midlife women, the route is especially important. Fatigue plus heavy bleeding, dizziness, fainting, shortness of breath, chest pain, or palpitations should not be managed as a supplement-shopping problem.
Vitamin D, magnesium, and calcium are not energy shortcuts
Vitamin D belongs in a fatigue conversation when the symptom is really bone pain, low sun exposure, malabsorption, osteoporosis risk, fall risk, or low measured vitamin D. NCBI Bookshelf notes adult vitamin D deficiency can contribute to osteoporosis, and that toxicity is possible with excessive oral intake. Excess vitamin D can cause hypercalcemia symptoms such as confusion, vomiting, frequent urination, excessive thirst, and muscle weakness. [4]
Magnesium is similar: it is essential, but deficiency context matters. MedlinePlus lists fatigue and weakness among early symptoms of a magnesium shortage, while also noting that it is rare to be truly lacking in magnesium. Risk is higher with gastrointestinal disease or surgery, type 2 diabetes, older age, and alcohol use. [5]
Calcium is often bundled into menopause supplements because bone health matters after menopause. That does not make calcium an energy treatment. Vitamin D and calcium may be part of a bone-density plan, but the dose should account for diet, kidney-stone history, kidney disease, other medicines, and total supplemental intake.
Where hormone therapy fits, and where it does not
If fatigue is mainly downstream of hot flashes and night sweats, treating vasomotor symptoms may improve sleep and daytime function. That is a different question from vitamin replacement.
Hormone therapy is not a fatigue vitamin, and it should not be used as a broad answer for low energy without a symptom target and contraindication screen. A better menopause visit asks which symptom is driving fatigue: hot flashes, insomnia, mood, vaginal or urinary symptoms, pain, bleeding, medication effects, metabolic changes, or under-fueling.

What to ask a clinician before buying supplements
Ask:
- Does my fatigue pattern look more like sleep disruption, hot flashes, anemia, thyroid disease, depression, sleep apnea, medication effect, under-eating, or a nutrient deficiency?
- Should we check complete blood count, ferritin or iron studies, vitamin B12, methylmalonic acid, vitamin D, thyroid-stimulating hormone, glucose, kidney function, liver tests, or another lab based on my symptoms?
- Do metformin, histamine-2 receptor blockers, proton pump inhibitors, blood-pressure medicines, antidepressants, glucagon-like peptide-1 medicines, alcohol, or other medicines change the plan?
- If a supplement is appropriate, what dose, duration, and recheck plan should I use?
- Which ingredients should I avoid because of kidney disease, kidney stones, constipation, medication interactions, excess iron risk, or duplicate nutrients from multiple products?
- If hot flashes or night sweats are the real sleep driver, should we discuss hormone therapy or nonhormonal vasomotor-symptom options instead of vitamins?
The right plan may still include a supplement. The difference is that it is targeted and measurable.
Bottom line
Vitamins for menopause fatigue are most useful when they replace something that is actually low: vitamin B12, iron, vitamin D, magnesium, or another nutrient tied to diet, absorption, blood loss, medication, or lab results.
They are not a universal energy treatment for menopause. If fatigue is coming from sleep disruption, hot flashes, thyroid disease, anemia, depression, sleep apnea, under-eating, medication effects, or heavy bleeding, the best next step is diagnosis first and supplements second.
